From Detox to Stability at 5450 West Sahara: PHP Beds, Housing, and Recovery Support in One Place

Leaving detox is supposed to be a fresh start. For too many people in Las Vegas, it's a return to the street, a couch that isn't safe, or no plan at all.

At 5450 West Sahara Avenue, partners are working to change that. Our lead partner, Together We Give, has Partial Hospitalization Program (PHP) beds open for adults with substance use disorder (SUD), and treatment partner Hope Harbor of Las Vegas provides SUD treatment and recovery support. Everything else a person needs to get stable — an address, an ID, a case manager, a job lead — is available in the same place.

One Building, One Plan

No single organization can solve addiction, homelessness, and unemployment alone. 5450 brings partners together so a person doesn't have to bounce between offices across the valley.

Who Should Be Referred to PHP

PHP is an outpatient care program. It's a strong fit for adults who:

  • Just finished detox and need daily structure before outpatient care
  • Face a high risk of withdrawal or relapse
  • Live with both a substance use disorder and a mental health condition
  • Are leaving jail, are court-referred, or are on probation or parole
  • Don't have stable housing that supports early recovery

Enrollment and approval are required

Every client must enroll in the program and be approved by the Program Director for bed placement. Service availability and eligibility vary by provider, and our team will help identify the right next step for each person.

How the Partnership Works

Together We Give

Together We Give leads the 5450 collaboration. It's a Joint Commission–accredited 501(c)(3) nonprofit that manages PHP bed placement, housing for enrolled clients, case management, and stabilization services.

Hope Harbor of Las Vegas

Hope Harbor is a BHCEN-certified outpatient and co-occurring disorder treatment center at 5450 West Sahara. It provides SUD treatment and the step-down from PHP into its Intensive Outpatient Program and outpatient care.

For Referral Partners: How to Send a Client

Detox units, hospitals, ERs, residential programs, case managers, jails, courts, and probation officers can all refer.

  1. Call Together We Give at (702) 518-3636 or email info@twgnv.org for PHP beds and housing.
  2. Share the basics: level of care, discharge or release date, co-occurring conditions, and Nevada Medicaid details.
  3. Eligibility review: The client enrolls and the Program Director reviews them for bed placement.
  4. Intake: Once approved, intake is often same-day or next-day.

Nevada Medicaid is accepted for PHP. Share records only through secure channels and with the client's written consent.
